For your convenience you may wish to print and complete the necessary paperwork in advance.  If you choose to complete the forms in advance you will be asked to arrive approximately 15 minutes prior to your actual appointment time.  If you prefer to do everything upon your arrival you will be asked to report 1/2 hour early. 

Please read carefully and answer all questions to the best of your ability.  If you have any questions please ask for assistance.  When you arrive one of our Administrative Assistants will use the information provided on these forms to create your dental chart. 

You will be asked to bring a list of any/all medications and herbs that you take and  the dosage.  Your medical history will be taken initially and reviewed at each subsequent visit. 

If you have dental insurance we will need a copy of your dental insurance card and a completely filled out and signed insurance claim form for your file.  If you do not have an insurance claim form on hand you should be able to obtain one from your employers human resource department or you may be able to download one on-line from your insurance company's website.
